Woman who fought flesh-eating illness goes home

After more than three months in the hospital and a rehabilitation clinic, a young Georgia woman who survived a rare fleshing-eating disease returned home to a family dinner and a wheelchair-accessible house now equipped with an exercise room and private elevator.

Aimee Copeland, 24, had been away from her family's home in Snellville outside Atlanta since May 4, when doctors diagnosed her with the life-threatening infection she contracted after gashing her leg while zip lining over the Tallapoosa River. Both of Copeland's hands, her left leg and her right foot were amputated at a hospital in Augusta. Then Copeland moved to the Shepherd Center in Atlanta for nearly two months of learning to move, eat, bathe and care for herself without prosthetic limbs.

With two trips in the family car to haul all of her things, and the bumper nearly dragging on the ground, Copeland moved back home Wednesday, her father said.

"It was almost like you were bringing a kid home from college," Andy Copeland said in a phone interview Thursday. "When we got back home, Aimee rolled around in her room and she was really laughing it up."

She returned to a home that had received a major makeover. A homebuilder, Pulte Homes, added a 1,956-square-foot living space for Copeland onto her parents' home. It includes a bedroom and bathroom with wheelchair-accessible features; wider doorways; an exercise room with parallel bars and other equipment; and an elevator between the addition's two floors. The $200,000 job was paid for by donations, at no cost to the Copeland family.

Copeland also got her first dinner outing and chose the steakhouse where her sister works, her father said. She dined on shrimp stew and spinach dip.

"She's actually very successful and is able to get around a good bit," Copeland's father said. "Last night she never wore a prosthetic limb the whole day. She was eating and drinking and taking care of herself without ever putting a prosthetic on."

He said his daughter is still adjusting to using prosthetic limbs and often finds them uncomfortably hot and itchy. Even without them, she's learning to pick up and use pens and other utensils by pressing her arms together to grip them. She also has special cuffs that fit on the ends of her arms that can hold a toothbrush, a fork or a spoon.

Copeland was pursuing a graduate degree in psychology at the University of West Georgia when she was injured during an outing with friends in May. She fell from a broken zip line onto sharp rocks along the Tallapoosa River and gashed her leg. Days later, doctors diagnosed Copeland with necrotizing fasciitis, a bacterial infection that emits toxins that can destroy muscle, fat and skin tissue.

Copeland defied doctors' expectations with a rapid recovery. Her father says they at first gave her just a slim chance of surviving, and she spent weeks sedated and breathing on a respirator.

Andy Copeland said it's unclear when his daughter will be able to return to school to finish her graduate thesis. The campus is 75 miles from her parents' home.

For now, he said, Copeland is just happy to be home.

"Right now her mental condition is wonderful, her health is good and her energy level is high," Copeland's father said. "She's doing great."

Two dead, 8 wounded in shooting near N.Y.'s Empire State Building

NEW YORK (Reuters) - A man shot dead a former co-worker near New York's Empire State Building on Friday and was killed in return fire by police who also wounded bystanders, creating chaos and shocking commuters and tourists who witnessed the bloody scene outside the famous skyscraper.

The gunfire wounded eight bystanders, who were all expected to survive, city officials said. All the shooting took place on the sidewalk at the height of the tourist season next to one of the city's most popular attractions

Jeffrey Johnson, a fashion accessories designer disgruntled over being fired a year ago, shot a 41-year-old former co-worker three times at close range with a .45 caliber handgun, officials said.

As police closed in, Johnson fired on them. The police returned fire and killed him, Police Commissioner Raymond Kelly said.

Investigators were attempting to determine whether Johnson shot anyone beyond his initial target. Some of the surviving victims could have been hit by the two police officers who returned fire, Mayor Michael Bloomberg said.

Bloomberg described the wounded as being "grazed." Two of those wounded were women, the rest men. None were children or elderly, Kelly said.

Johnson, of Manhattan, had worked at Hazan Imports across the street from the skyscraper, Kelly said.

A white tarp covered Johnson's body in front of the entrance to the office building. There was no link to the historic tower -- it just happened to be next door -- and Bloomberg ruled out any connection to terrorism.

"I heard the gunshots. It was like pop, pop, pop. It was definitely in a bunch," said Dahlia Anister, 33, who works at an office near the 102-story building.

It was the third mass shooting of the summer in the United States, following an assault on a crowded cinema in Colorado and an attack on a Sikh temple in Wisconsin, rekindling debate about gun control in America. This shooting was distinct in that Johnson appeared to have only one intended victim.

DECLINING CRIME RATE

"We are not immune to the national problem of gun violence," said Bloomberg, a leading national proponent of gun control and founder of the group Mayors Against Illegal Guns.

Bloomberg has often called New York the safest big city in America, citing a declining crime rate that had the city on pace for another record low number of homicides in 2012.

"It's time to get the guns off the street," said Brandon Thorpe, 23, a janitor on his way home from work who said he has lost five friends to gun violence. "This is a tourist attraction. How are we supposed to make people feel safe if they come here and see something like this?"

The Empire State Building is walking distance from Pennsylvania Station and Grand Central Terminal, two of New York City's main transportation hubs, and the shooting took place shortly after 9 a.m. at the end of the morning rush hour.

Police cordoned off the area and closed several streets in the heavily transited area.

President Barack Obama was notified of the shooting shortly after 9:30 a.m. on Friday by his homeland security adviser John Brennan, and was continuing to receive updates, the White House said.

Mail courier James Bolden, 31, said he saw a "guy laying on the (sidewalk), bleeding from the neck and barely breathing."

"Everybody was crowded around him taking pictures and video, and security guys were yelling everybody to get back, and give him space. He was barely breathing," Bolden said.

One witness said she saw a woman who was shot in the foot and another woman being taken away in an ambulance.

"I was walking down 33rd (Street) and there's a dead guy. I just saw pools of blood. He was laying down and the was blood pooling (around him)," Justin Kellis, 35, who works nearby.

SUMMER MASS SHOOTINGS

The United States has had two other mass shooting cases this summer. On July 20, a gunman opened fire at a midnight screening of the Batman film "The Dark Knight Rises" in Aurora, Colorado, killing 12 people and wounding 58.

On August 5, a gunman killed six people and critically wounded three at a Sikh temple outside Milwaukee before police shot him dead in an attack authorities treated as an act of domestic terrorism.

This was the second high-profile shooting incident in two weeks in New York's tourist-heavy Midtown Manhattan. On August 12 New York City police shot and killed a knife-wielding suspect as he sought to evade them through Saturday afternoon traffic and pedestrians in Times Square.

The Empire State Building was the world's tallest building for 40 years from its completion in 1931 until construction of the World Trade Center in 1971.

Several skyscrapers around the world have since surpassed the New York buildings. Following the September 11, 2001, attacks that destroyed the World Trade Center, the Empire State Building was again the tallest building in New York City, though it was recently surpassed by a new tower under construction at Trade Center's site.

What Is Colorectal Cancer (Colon and Rectal Cancer)?

Colorectal cancer is a term used to refer to cancer that starts in the colon or rectum. Colon and rectal cancers begin in the digestive system, also called the GI (gastrointestinal) system. This is where food is processed to create energy and rid the body of solid waste matter (stool).

After food is chewed and swallowed, it travels down to the stomach. There it is partly broken down and sent to the small intestine. The word ?small? refers to the width of the small intestine. The small intestine is really the longest part of the digestive system. It is about 20 feet long.

The small intestine continues breaking down the food and absorbs most of the nutrients. The small intestine joins the large intestine (also called the large bowel or colon), a muscular tube about five feet long. The first part of the colon absorbs water and nutrients from the food and also serves as a storage place for waste matter. The waste matter moves from the colon into the rectum, the last 6 inches of the digestive system. From there the waste passes out of the body through the opening called the anus.

The colon has 4 sections, as shown in the picture above. Cancer can start in any of the 4 sections or in the rectum. The wall of each of these sections (and rectum) has several layers of tissues. Cancer starts in the inner layer and can grow through some or all of the other layers. Knowing a little about these layers is helpful because the stage (extent of spread) of a cancer depends to a great degree on which of these layers it affects.

Cancer that starts in the different areas may cause different symptoms. But colon cancer and rectal cancer, together known as colorectal cancer, have many features in common. They will be discussed together in this document except for the section about treatment. There they will each be discussed separately.

In most cases, colorectal cancers develop slowly over a period of several years. We now know that most of these cancers begin as a polyp?a growth of tissue that starts in the lining and grows into the center of the colon or rectum. A type of polyp known as an adenoma can become cancerous. Removing the polyp early may prevent it from becoming cancer.

Over 95% of colon and rectal cancers are adenocarcinomas. These are cancers of the cells that line the inside of the colon and rectum. There are some other, more rare, types of tumors of the colon and rectum, but the facts given here refer only to adenocarcinomas.

Causes of colorectal cancer

There is no single cause of colorectal cancer, but some factors appear to increase the risk of developing it : -1. age ? particularly after 50.2. having polyps (small growths on the inner wall of the colon and rectum).3. family history of colorectal cancer ? especially if the relative (parent, sibling, child) developed colorectal cancer before the age of 45.4. having familial adenomatous polyposis (FAP) or hereditary nonpolyposis colon cancer (HNCC).5. inflammatory bowel disease (ulcerative colitis or Crohn?s disease).6. high-fat diet.7. alcohol consumption.8. smoking.9. physical inactivity.10. obesity.11. ethnic background ? people of Ashkenazi (Eastern European Jewish) descent. Researchers are also looking at how diet affects the risk of developing colorectal cancer. A diet high in vegetables and fruit is known to lower risk. A diet high in fiber and low in animal fats also seems to decrease risk, but more research is necessary to be sure. Some people develop colorectal cancer without any of these risk factors.

Symptoms of Colorectal Cancer (Colon and Rectal Cancer)

The following are the most common symptoms of colorectal cancer. However, each person may experience symptoms differently : -1. A change in bowel habits-such as diarrhea, constipation, or narrowing of the stool-that lasts for more than a few days.2. Bright red or very dark blood in the stool.3. Discomfort in the stomach area, such as frequent gas pain, cramps, and feelings of being full or bloated.4. Constant weakness and fatigue.5. Jaundice ? yellowing of the skin and eyes.6. weight loss for no known reason.

The diagnosis of Colorectal Cancer (Colon and Rectal Cancer)

The first step in the diagnosis of colorectal cancer may be a ?digital rectal exam.? In this test, which is often part of a routine physical exam, the doctor inserts a gloved finger into the rectum to feel for abnormal growths and may test your stool for occult blood. If colorectal cancer is suspected, you and your doctor have many tests to choose from to make sure the diagnosis is correct. Since there is a wide range of options, knowing the differences can help you make a smart decision.

Before these tests (except the fecal occult blood test), your colon must be completely empty. To clean out your bowels before the procedure, you may be instructed to have an enema or drink a large volume of a liquid laxative prescribed by your doctor. This preparation is very important in ensuring the accuracy of the test.

Fecal occult blood test : ? In this simple, initial screening test for colorectal cancer, a stool sample is applied to a special card that turns color if any blood is detected.

Barium enema : ? A narrow tube is inserted into the rectum to allow liquid barium (a white, chalky liquid) into the colon. A special x-ray is then taken, on which tumors or masses appear as dark shadows. The barium makes it easier to see the tumors. Before this test, you will be told to fast (not eat or drink) for several hours.

Flexible sigmoidoscopy : ? A thin, lighted tube inserted into the rectum allows the doctor to look into the rectum and first section of the colon (where half of all polyps are found).

Colonoscopy : ? The most accurate and thorough test for colorectal cancer (colon and rectal cancer). A long, thin, lighted tube inserted into the rectum allows the doctor to look into the entire colon, remove polyps, and take tissue for biopsy. Polyp removal will prevent cancer from developing. A mild sedative is usually given with this test.

Ultrasound : ? This noninvasive test uses images and sound waves to take a picture of the inside of the body. Unusual patterns in this image can indicate a mass or tumor. This test is used primarily to find cancer that has already spread to other organs.

Virtual colonoscopy (CT colonography) : ? This test creates a 3-dimensional reconstruction of the colon to reveal any abnormal masses. Images are taken seconds after the colon is inflated with carbon dioxide through a small rectal tube. Virtual colonoscopy is a fairly new technology, so it is not yet clear how accurate it is.

Treatment options for Colorectal Cancer in India : ? Treatment options for colorectal cancer depend on the stage of the tumor-that is, how far it has spread or how deeply it is affecting the intestinal wall and other tissues. Treatment is also determined by the patient?s age, medical history, overall health, and tolerance for specific medications and therapies. Standard options for colorectal cancer treatment in India include : -

1. Partial colectomy for colorectal cancer in India (also called partial bowel resection)-The tumor and normal tissue on either side of the diseased area in the colon are removed. The surgeon then reconnects the healthy colon. Sometimes the physician may have to create a temporary colostomy, or an opening for solid waste from the bowel to a special bag a patient wears outside the body, until the healthy tissue has healed. At times, the colostomy is permanent.2. Laparoscopic surgery or keyhole surgery for colorectal cancer in India -Small tube-like instruments and an extremely small camera are inserted into the abdomen through incisions made in the abdominal wall. The surgeon sees what the camera sees on a television-type screen and can cut out a large section of the bowel and adjacent tissue, called the mesentery.3. Radiation therapy for colorectal cancerin India
